Clinical Records
According to the HIPAA Privacy Rule, I have the obligation to safeguard the PHI that is kept in your clinical record and you have the right

to access it. Should you request information from your record, I will typically provide a summary of the record which accurately reflects the 

record's contents.  I may provide the summary, rather than the record itself, if I believe the effect would be harmful to you or others.  Otherwise,

I typically would ask that you review the record in my presence and that we discuss it together. 
Patient Rights
You have the right to revoke, restrict or limit your written authorization allowing disclosure of PHI through a letter to that effect.

You have the right to receive all correspondence from me at the location and through the means you specify.

You have the right to receive an accounting of PHI disclosures for which you have neither provided consent nor authorization. However, it is 
not my policy to release PHI without your knowledge.

You have the right to add amendments to your clinical record to correct information you believe is inaccurate. The record may be amended but
the information will not be changed or deleted.
